Welcome! Thank you for your interest in getting a Lebanon Libraries card.

Please note: Provisional cards are for NEW patrons only. If you have a current Lebanon Libraries card or have had one in the past, come on in or call the library (603-448-2459 or 603-298-8544) and we’ll renew your previous card or help you access your account. Do not fill out this form.

New cardholders: after filling out the form below, come in to either library (Lebanon Library, 9 East Park Street, Lebanon OR Kilton Library, 80 Main Street, West Lebanon). Bring proof of residency with you and we’ll give you a card and let you know about all of the services, materials, and programs we offer. Proof of address may be in the form of a valid driver’s license, received and USPO-cancelled mail, rent receipts, utility bills, tax receipts, or any such items showing the applicant’s name and street address.

The following are eligible for a free Lebanon Libraries card:

  • Any resident of the City of Lebanon.

  • Any teacher or student in the Lebanon public schools.

  • Any Lebanon business or nonprofit agency will be issued a card in the name of the owner or CEO. The owner or CEO is responsible for all items borrowed. The card must be presented when borrowing.

  • Any student of a college located in Lebanon currently enrolled and in good standing.

NON-RESIDENT CARDS: Any person residing in another community and not meeting the conditions above may obtain a library card upon the completion of registration. The non-resident fee is $75.00 per year per family. A six-month card may be issued at a cost of $40.00. Senior citizens may obtain their cards for $60.00 per year or $35.00 per 6 months. Senior residents of Hartford, VT (age 60 and over) and residents with a child in fifth grade or under may get a non-resident card for their household for $20.00 annually.
